Position Summary
The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care.
Position: 32 hours a week/4-day work week. Must be able to work 1 or 2 weekend days a month.
Position located in North Central Florida, Gainesville area.
Essential Functions
Provide medical expertise in pain and symptom management, including terminal prognosis evaluation.
Act as attending provider in hospice contract beds in collaboration with the hospice physician.
Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses.
Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes.
Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings.
Perform physical exams, diagnostic testing, treatment, follow-up, and consultations.
Prescribe and administer medications and other therapeutic interventions.
Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team.
Support patients and families in clarifying care goals and end-of-life decisions.
Maintain accurate and complete medical records to ensure compliance and reimbursement.
Ensure informed consent documentation, including CPR and DNR discussions.
Participate in interdisciplinary team meetings and on-call rotations.
Educate internal staff and community members on palliative and hospice care.
Coordinate inpatient APRN assignments and support departmental functions.
Qualifications
Knowledge and Critical Skills
Expertise in patient charting, history taking, and documentation practices.
Knowledge of health education methods, clinical operations, and medical procedures.
Proficient in diagnosis, treatment planning, and symptom management.
Excellent interpersonal, verbal, and written communication skills.
Ability to organize, analyze, and prioritize complex tasks and data.
Comfortable with public speaking, leadership, and team collaboration.
Experience
Minimum two (2) years of nursing experience.
Experience in hospice or palliative care preferred.
License / Certification
Current Advanced Practice Nurse or Physician Assistant license in the State of Florida.
Valid Florida driver’s license with acceptable driving record and reliable transportation.
Education
Master’s degree from an accredited Advanced Practice Nursing program.
Other Requirements
Medical clearance for communicable diseases prior to patient contact.
Up-to-date immunizations including, but not limited to, TB, Hepatitis, and Influenza.
