Position Summary

VNA Hospice NWI is seeking a dedicated Nurse Practitioner to provide high-quality palliative and hospice care across home settings, long-term care facilities, and our inpatient hospice center. This role is ideal for an NP who values relationship-based care, autonomy, and a supportive team environment.


Key Responsibilities

  • Perform comprehensive palliative and hospice assessments
  • Manage symptoms including pain, dyspnea, anxiety, nausea, and agitation
  • Collaborate closely with hospice physicians, RNs, social workers, chaplains, and volunteers
  • Participate in interdisciplinary team meetings and care planning
  • Provide education and emotional support to patients and families
  • Assist with advance care planning and goals-of-care discussions
  • Document all encounters in the EMR in accordance with regulatory standards
  • Support transitions of care and coordinate with community partners
Requirements

 Qualifications

  • Master’s or Doctorate in Nursing
  • Current Indiana Nurse Practitioner license
  • Board certification (AGNP, FNP, or related specialty)
  • Hospice, palliative care, or geriatrics experience preferred
  • Valid driver’s license and reliable transportation
  • Strong communication skills and a compassionate, patient-centered approach
